3NT is a 2 point range for me, and there is no point in quantitative.
Presumably the 2♦ is not a negative.
4NT after a transfer to hearts is ace asking if that is your ask, but I would prefer it to be a heart slam invitation if opener has 3 or 4 card support - opener responding with aces if so, passing if not. If you want to ace ask regardless, use kickback 4♠.
Baron being a request for 4 card suits upwards? After Baron, 4NT has to be to be straight ace asking, I would have thought, as presumably you would bid 4♠ looking for a 4-4 fit game there, and would not use Baron unless you had a 4 card major.

What is responder supposed to do if he was 4-4 in the reds and was just interested if he found a fit? I would continue:
4NT Nat
5m Nat presumably 4♥ 6m invitational.
5♥ Some sort of try
5♠ Some sort of try
5NT RKCB for spades (had partner bid 4♥, 5♠ would be RKCB, and so on)
Those are clearly not perfect, but fit in with my generic rules, hence no fancy meaning for 5♥.

You are in an almost impossible situation. You could play that partner only transfers with with slam interest, but this causes problems of it's own. Better is to change the structure:
4♣ Baron
4♦ Sign-off in either major or various (strong major suit slam tries start this way)
4♥/♠ encouraging
This structure also applies when we overcall 3NT, so it's worth investing time in.

I prefer to play the following in reply to 3NT:
4C - Gerber
4D, 4H - transfers to 4H and 4S respectively.
4S - transfer to Cs showing 5+ Cs.
4NT - Quantiative raise.
5C - transfer to 5D showing 5+ Ds.
4C - Gerber
