1S-(Pass)-1NT-(2NT)

2NT for the minors.
1NT was not forcing, if if it matters.
Do you play U/U is on here?

As it happened, I had a fairly modest but not awful hand with five hearts and a stiff spade so, when opener now bid 3H, I raised to 4H, figuring how bad can it be?
Making 5, partly from helpful defense.

This situation is certainly different from 1S-(2NT). But still...
I can't say I ever much thought about this before, I don't recall it coming up.

It is true that you could play something like good/bad:

3 = 5+ & 4+, gameforcing
3 = 6+, gameforcing
3M = natural, not forcing

but in reality I would probably never bid on the 3-level with a hand where partner should not raise to game with a fit, so it wouldn't add much.
