Ruff it, if you had A♦ stiff and more than one heart, wouldn't you lead it to get your ruff ?

Play a trump, if trumps are 2-1 and W is 4225/4135 you're in good shape here. If E had 6♠/5♦ he wouldn't have stood the double and would have bid 5♠, I suspect W has 1098x, Ax, AJ, AJxxx or similar.

Yeah, and if East does not have the jack of ♦, a tad embarrassing if declarer has QJ of ♦ and partner has to ruff with say doubleton KQ of trumps instead of putting up the king and shoot a club back through declarer.
